The RN Care Manager is responsible for the delivery of high-quality, patient-centered, longitudinal clinical care management, education and coordination to achieve optimal patient health outcomes for patients who are medically homed within UVM Medical Center Primary Care.  The patient population will include significantly complex medical conditions, and/or social – economic co-morbidities.  The goal of the program will be to support these patients in achieving their optimal health goals and/or independence in managing their care.  To achieve this goal, the RN Care Manager will demonstrate and apply knowledge of the philosophy/principles of comprehensive care management, patient-centered, culturally sensitive care coordination and management of complex patients.

 

In collaboration with the care team, the RN Care Manager will be responsible for developing plans for patient and family self-care competence, using motivational interviewing, assessing for desired level of involvement and coaching for adherence to care plan. This individual will provide nursing assessment, create and monitor patient/family care plans, establish care goals and shared care plans. The primary contact with the patient, family and other involved care providers will be conducted via telephone, in-person visits/assessments and electronic medical record (EMR).

 

The RN Care Manager serves as an ambassador of the Medical Home Care Management program throughout the organization, within the community and within additional hospitals and programs where UVM Medical Center medically homed patients live and receive care.  In collaboration with Outpatient Social Work, they are responsible for creating and sustaining partnerships with physicians/providers, staff, community resources and support agencies.

 

The RN Care Manager works with health care providers (both inside and external to the Medical Center), community-based patient supports, Accountable Care Organization(s), insurers, and appropriate governmental entities.

 

EDUCATION:

State of Vermont Registered Nurse (RN) license required. The University of Vermont Medical Center is moving toward an all Bachelor of Science in Nursing (BSN) workforce. All external candidates hired into, or current employees promoted to, a RN position will be required to obtain their BSN as outlined in the terms of their offer letter.  Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.

 

CCMC (Commission for Case Manager Certification) or CCCTM (Certification in Care Coordination and Transition Management) or equivalent certification required, or must be obtained within 24 months of hire.

 

Must have and maintain a valid driver’s license.

 

EXPERIENCE:

Minimum of 2 years relevant RN clinical experience. Clinical experience in nursing care of the medically complex patient highly desirable.
